Joshua Bryant

Joshua Bryant, CNP
Joshua Bryant is a certified Family Nurse Practitioner certified through the American Academy of Nurse Practitioners. He received a bachelor's degree in Biology and Master's degree in Entomology prior to earning a Master's of Science in Nursing all from The Ohio State University. Prior to becoming a nurse Practitioner, Joshua spent 5 years working as an entomologist at Ohio State University where he studied the effects of pesticides on household and structural pests. Once he obtained his RN license, Joshua worked as a staff nurse on a medical-surgical unit at The Ohio State University Wexner Medical Center in Columbus. Joshua completed a student rotation at Community Health and Wellness Partners of Logan County and has a strong interest rural healthcare including health promotion, disease prevention, and management of chronic health conditions. Joshua grew up in a Pike County, Ohio, in Piketon. Joshua currently lives in Columbus Ohio, but will be moving to Marysville in July. He enjoys hiking, gardening, and cooking.

Erinn Dennis-DaCunha

Erinn Dennis-DaCunha, LPC-C
Erinn Dennis-DaCunha is a Licensed Professional Clinical Counselor. She received her Bachelors of Fine Arts Degree at Bradford College in 1992 and her Masters in Clinical Mental Health Counseling from Ashland Seminary in 2015. Erinn has been an employee of The Ohio State University in Columbus since 2007. Erinn was introduced to CHWPLC through OSU College of Nursing in 2016 through the MedTapp Healthcare Access initiative Program. Erinn was later hired on as a counselor in 2017 by CHWPLC. Prior to being hired on by CHWPLC she worked in Marion Ohio where the majority of her clients were children whose families had been directly impacted by the opiate epidemic. Erinn presently splits her time working for CHWPLC and The College of Nursing at OSU East as a Mental Health Counselor. Erinn is married and has two adult children and resides in Franklin County. When not working she enjoys running, being involved in her church community, spending time with family, and traveling.

William DeGendt

William DeGendt, CNP
William DeGendt, CNP, is a Family Nurse Practitioner, board certified through the American Academy of Nurse Practitioners. A lifelong Ohioan, he was born in Coshocton and grew up in the Zanesville area. William attended the Ohio State University, graduating with a Bachelor's of Science in Nursing and then a Master's of Science in the Family Nurse Practitioner specialty. In the interim, he worked as a registered nurse at Mount Carmel West, and then as a certified emergency nurse at Sycamore Medical Center for five years. William's professional interests include wellness-focused care of patients of all ages and their families, as well as management of acute and chronic conditions. William and his wife, Lindsey, enjoy cooking, hiking, and playing with their two golden retrievers.

Breanna

Breanna Detrick, CNP
Breanna Detrick is a certified Family Nurse Practitioner certified through the American Association of Nurse Practitioners. She received her Bachelors of Science in Biological Sciences from the University of Toledo and her Masters of Science in Nursing from The Ohio State University. Prior to becoming a Nurse Practitioner, she was involved in clinical research at Nationwide Children's Hospital and the Ohio State Wexner Medical Center. Breanna spent multiple student rotations at Community Health and Wellness Partners of Logan County and has been serving patients of this community ever since. She welcomes patients of all ages and enjoys being able to take care of the entire family. Her focus is on health promotion and disease prevention, as well as acute and chronic disease management. Breanna and her husband, Jeff, currently reside in Westerville. They are on a weekly bowling league and love to explore the National Parks.

Gildow

Justin Gildow, CNP
Justin Gildow, CNP, was born and raised in Bellefontaine, Ohio and is board certified as a Family Nurse Practitioner through the American Nursing Credentialing Center. Justin earned his Bachelor's of Science in Nursing degree at Cedarville University in 2007 and his Master of Science in Nursing degree from the University of Cincinnati in 2012. Prior to becoming a Nurse Practitioner, Justin worked as a nurse for several years with advanced care and intensive care patients at Miami Valley Hospital in Dayton, Ohio. Justin enjoys caring for the whole person-including physical, mental, emotional & spiritual aspects. He has had a wide range of specialized experience during his time as a Nurse Practitioner and is specifically focused on health promotion and disease prevention as well as caring for acute and chronic illnesses in patients of all ages. Justin lives in Logan County with his wife, Andrea, and their three children where they enjoy sports and spending time outdoors.

Charles Kratz

Charles Kratz MD
Dr. Charles Kratz joined Community Health & Wellness Partners of Logan County on March 1, 2016, after caring for patients at Oakhill Medical Associates for 20 years. Dr. Kratz has served as the President of the Medical Staff at Mary Rutan Hospital and is currently the Medical Director for Hospice Services with Universal Home Health and Hospice. A Medical Degree recipient from Temple University School of Medicine, Dr. Kratz also attended Eastern Mennonite University where he received a Bachelor's Degree in Biology with minors in English and Chemistry. He went on to complete his residency at Williamsport Hospital Family Practice Residency Program Williamsport, PA. A member of the American Academy of Family Practice, Christian Medical & Dental and the Mennonite Medical Associations, Dr. Kratz has been Board Certified since 1996. He is married to Patricia Kratz and they have five children, residing in rural Logan County.

Amanda

Amanda Lollini, CNP
Amanda Lollini, CNP is originally from Brilliant, Ohio, a small town on the Ohio-West Virginia border. She attended The Ohio State University to obtain both her Bachelor's of Science in Nursing degree and Master of Science degree with a Family Nurse Practitioner specialty. She is a board certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. Prior to becoming a Nurse Practitioner, Amanda worked as a nurse for several years with oncology and transplant patients at Ohio State. Amanda's specific interests include women's health, geriatric care, chronic illnesses, and preventive medicine. She recently joined CHWPLC as a new provider and is excited to be practicing in Logan County. Amanda resides in Union County, where she enjoys biking, being outdoors, and spending time with her husband and dog.

Jason Martinez

Jason Martinez, PharmD, BCACP
Jason Martinez is originally from Pittsburgh but moved to Ohio to attend Ohio Northern University where he received his Doctor of Pharmacy degree in 2009. Jason is a Certified Ambulatory Care Pharmacist through the Board of Pharmacy Specialties. Experienced in retail and hospital sector with an interest in chronic disease management, Jason works collaboratively with our other providers to optimize treatment plans in addition to helping patients understand how to take their medications or find more affordable options. Residing in Union County with his wife and two children, Jason enjoy sports and spending time outdoors.


Kennth Miller

Kenneth Miller MD
Dr. Kennth Miller also joined Community Health & Wellness Partners of Logan County on March 1, 2016, after caring for patients at Oakhill Medical Associates for nearly 28 years. Dr. Miller has served as the President of the Medical Staff at Mary Rutan Hospital and is currently a member of the Board of Directors of the Medical Group of Ohio. A Medical Degree recipient from The Ohio State University College of Medicine, Dr. Miller also attended Goshen College where he received a Bachelor's Degree in Biology. He went on to complete his residency at Grant Family Medicine Residency Program in Columbus, OH. A member of the American Academy of Family Practice, Christian & Dental and the Medical Mennonite Associations, Dr. Miller has been Board Certified since 1988. Dr. Miller and his wife, Doris, live in Bellefontaine and have three grown daughters.

Brian Schmidt

Brian Schmidt, DO
Dr. Brian L. Schmidt, a Preble County native returned to Ohio after serving in the United States Navy for 11 years. A graduate from Mount Vernon Nazarene University, Dr. Schmidt received his Osteopathic Medicine degree in 2006. A transitional internship year was spent in surgery at the Naval Medical center in San Diego, CA. Transferring to Hawaii, Dr. Schmidt went on to serve with the United States Marines Infantry 2nd Battalion, 3rd Marine Regiment. While in the service to the Marines, Dr. Schmidt was deployed to Fallujah, Iraq in 2008 and to Helmand Province, Afghanistan in 2009.

Dr. Schmidt returned to the United States Navy in 2010 and completed his Family Medicine residency training at the U.S. Naval Hospital in Pensacola, FL, serving as Chief Resident. Just prior to joining CHWPLC, Dr. Schmidt served at the U.S. Naval Air Station hospital in Lemoore, CA where he was elected Chair of the Executive Committee for Medical Staff. A board certified physician with the American Board of Family Medicine, Dr. Schmidt sees patients of all ages for primary care (excluding obstetrical care).

An avid outdoorsman, Dr. Schmidt, his wife and five children reside in rural Logan County.


Jillian Shellabarger

Jillian Shellabarger, MD, Psychiatrist
Dr. Shellabarger attended Otterbein University where she received a Bachelor's Degree in Life Science with a minor in Sociology and graduated with honors in 2010. She then attended Wright State University Boonshoft School of Medicine and was the recipient of a medical degree in 2010. She then completed a residency in psychiatry through Wright State University in Dayton, OH, in 2014. This was followed by a fellowship in child and adolescent psychiatry through Wright State University in Dayton, OH, in 2015, serving as Chief Resident. She maintains board certification in both adult and child and adolescent psychiatry, and is a member of the American Psychiatric Association and the Ohio Psychiatric Physicians Association. Born and raised in rural Ohio, she remains committed to providing psychiatric care to rural populations, and has joined Community Health & Wellness Partners of Logan County to broaden access to psychiatric care.


Laura Shoemaker

Laura Shoemaker, LISW-S
Lara Shoemaker is a Licensed Independent Social Worker Supervisor who is Board Certified through the Counselor, Social Worker, Marriage and Family Therapist Board of Ohio. She received her B.A. in Social Work from Eastern Mennonite University in Harrisonburg, VA and her Masters of Clinical Social Work from The University of Denver, in Denver, CO. Lara began her professional career at Adriel School, Inc, working with youth in foster care and group home settings who had severe emotional and behavioral issues. She was promoted to Director of Clinical Services at Adriel in 2015, serving there until joining Community Health and Wellness Partners of Logan County. Lara is excited about working with kids, teens, and adults in collaboration with their Primary Care Provider to address mental health needs, such as Anxiety, Depression, Stress, and ADHD. Originally from Archbold, OH, Lara lives in West Liberty with her husband, Josh, their son Luke, and dog Piper. Lara enjoys spending time with her family, reading, and trying new recipes.


Leigh Ann Swank, CNP

Leigh Ann Swank, CNP
Leigh Ann Swank, CNP, was born and raised in West Liberty, Ohio and is board certified as a Family Nurse Practitioner through the American Association of Nurse Practitioners. She received her degree in nursing from Kettering College of Medical Arts in 2008. She then went on to receive her Masters of Science in Nursing Administration from the University of Phoenix and her Masters of Science degree from Ohio University with a specialty in Family Nurse Practitioner. She is coming to CHWPLC after working as a CNP for Pulmonary & Critical Care Consultants in Dayton, Ohio. Prior to becoming a Nurse Practitioner, Leigh Ann worked for Premier Health as an Advanced Care, PACU, and Emergency Department RN. She enjoys taking care of all patients throughout their lifespan with a focus on health promotion and disease prevention, as well as acute and chronic disease management. Leigh Ann lives in Champaign County with her husband, Brad and their two children. She enjoys spending time with her family, reading, and gardening.


Onalee Volio

Onalee Volio, MS, RD, LD
Onalee Volio grew up in Avon Lake, Ohio near Cleveland, and moved to Columbus to attend college. Onalee earned her bachelor's degree in Medical Dietetics and her master's degree in Human Nutrition and Food Management, both from The Ohio State University. She has worked as a dietitian in both a food service management and a clinical nutrition capacity and has practiced in a variety of settings, including hospitals, long term care, and long term acute care. Working at CHWPLC in a teaching/counseling environment is an exciting change for Onalee, where she is committed to helping patients learn how important dietary changes can be in improving overall health. Onalee has two grown children, a daughter and a son. Her husband, Jon, runs his own lawn care company. They have resided in their traditional country home in Union County between Marysville and Bellefontaine for many years, and appreciate the rural beauty and small town atmosphere that this area provides. When not working, Onalee and Jon enjoy relaxing at home as well as boating, water sports, gardening and cooking.
