Community means Everyone. CHWPLC offers a full-range of healthcare services at two locations, West Liberty and Russells Point, and serves all persons--the insured, uninsured*, underinsured, and self-pay. (A reduced payment system, known as a Sliding Fee Scale may be available based on household income.) *Because we are able to offer care for a broader range of health insurance coverages, CHWPLC provides additional assistance to uninsured patients to enroll in Medicaid, CHIP, and other programs.
Community is at the Core. As a federally-qualified health center, CHWPLC has joined a network of over 40 health centers in Ohio and over 1,800 nationally, all committed to serving citizens in their respective communities. We work with Mary Rutan Hospital to reduce the burden on their emergency room through the implementation of patient care programs which ultimately improves patient health outcomes and reduces costs associated with the treatment of chronic illnesses.
Community has a Voice. Through a Board of Directors comprised of citizens of Logan County, there is A COMMUNITY VOICE in the operation of CHWPLC which allows the organization to be guided by consumers' opinions, thoughts, and ideas.
Community Investment. We are focused on providing the best healthcare possible and we recognize that in order to do so, we must also invest in our community through economic development and the growth of our organization. Future plans include a new site location in Bellefontaine and expanded services at the Indian Lake Community Health Center site.
